Veterinary Referral & Consent

All physiotherapy treatment is carried out in accordance with the Veterinary Surgeons Act 1966.
Animals must be under the care of a registered veterinary surgeon, and veterinary consent must be obtained prior to treatment.

Referral Requirements

• A completed veterinary referral or written consent is required prior to treatment

• Referrals may be submitted directly by the veterinary practice or via the owner with veterinary approval

• Referrals can be completed using the form above or provided in writing via email
